Know your plan

Whether obtaining a new plan or renewing an existing plan, consumers are encouraged to find the option best-suited for their unique needs.
Questions to ask before enrolling or changing an existing plan:

  1. Will I be able to see my health care providers and are they in the plan’s network?
  2. If my providers aren’t in the network, will the health plan still cover my visits if I choose to see them?
  3. Which specialists, hospitals, home health agencies, and skilled nursing facilities are in the plan’s network?
  4. How much is my monthly premium?
  5. How much will it cost to see my health care provider or a specialist?
  6. Does this plan require authorization or referrals for procedures/services?
  7. Are my prescription drugs on the plan’s formulary?
  8. Do I have to pay a deductible before the plan will cover my drugs?
  9. Will the plan cover my prescriptions when I travel?
  10. What are the annual deductible out-of-pocket limits for in-network and out-of-network care?
  11. What service area does the plan cover and what kind of coverage do I have if I travel outside of the service area?
  12. How does the plan compare to my current plan or other plans?
