Medical Records

Wickenburg Community Hospital & Clinic’s Health Information Management department (HIM) is responsible for keeping complete medical records for each patient.

The HIM office is located inside the hospital, enter through the Bralliar Entrance.

Office Hours | Monday – Friday 8:00 am – 4:30 pm.

Copies of your medical record information can be requested by contacting a representative at 928-684-4364 or by faxing a request to 928-684-9406.

Prior to receiving the copies, a standard “Authorization to Disclose Health Information” form must be completed and proof of identification provided. Requests can be emailed with completed Release of Informed Consent forms to medical.records@wickhosp.com.
