How to Become a Patient

We’ve made the process to become a patient at the Boyd Infusion Center as smooth and straightforward as possible.

Steps to Receive IV Infusion Therapy in the Boyd Infusion Center

  1. Provider Referral

Your healthcare provider will contact us and fax an order for treatment. The order must include your diagnosis, data sheet, office notes, and lab results.

  1. Insurance Authorization

Once we receive your provider’s order, we handle the insurance authorization process for you.

  1. Appointment Scheduling

After obtaining authorization, our team will contact you to schedule your appointment at a convenient time. For answers to your other questions, please call our out-patient hospital scheduling line at 928-668-1803.

IV Infusion Therapy When and Where You Need Them

For Patients with Out-of-State Providers

We are happy to assist patients who have out-of-state providers. However, Arizona law requires us to follow state guidelines, meaning we must obtain an order signed by an Arizona-licensed provider for treatment. Our staff will guide you through this process seamlessly.
