How do I become an infusion patient?
Your provider will contact us and fax an order with diagnosis, data sheet, office notes, and labs. Once received, we conduct insurance authorization and when obtained, we contact you to schedule an appointment.
What if I have an out-of-state provider?
We will assist you in obtaining an Arizona licensed provider order as we are unable to accept orders with out-of-state provider signatures.
