Every three years, we conduct a Community Health Needs Assessment (CHNA) to gain valuable insights into how we can better serve you. For 2025, we’re introducing new ways for you to share your voice—and make a real difference.

  • Format: A short Zoom session, making participation easy and convenient.
  • Group Size: Limited to 5-6 individuals to ensure meaningful discussions.
